javascript - How to set dynamic form on popup -

i have dynamic form code following:

i want show on popup in wordpress issue not supporting javascript using ninja popup , popup builder. please suggest me plugin support or have create own popup.

wordpress has built in routines enqueue javascript , css, best insert below theme's function.php

note have make sure add necessary css plugin, either save in theme css folder , call my_extra.css or change code above suit loading css external web page.
